Are you a Qualified Social Worker with a strong background in adult mental health and hospital discharge? We’re looking for experienced and motivated practitioners to join a Mental Health & Hospital Discharge Team in Cheshire, supporting adults to live safely, independently, and with dignity. You’ll be working across community and hospital settings, ensuring that people receive timely, strengths-based support tailored to their needs. 🌟 What You’ll Be Doing:

Completing Care Act assessments to support safe and timely hospital discharges
Carrying out mental capacity assessments and contributing to best interest decisions
Managing a caseload including safeguarding enquiries, complex care planning, and reviews
Liaising with hospitals, community services, families, and other professionals
Ensuring service users receive holistic, appropriate, and empowering support

✅ Who We’re Looking For:

Qualified Social Worker registered with Social Work England
Proven experience in Adult Social Care, ideally in mental health or hospital discharge teams
Strong working knowledge of:
- Care Act 2014
- Mental Capacity Act 2005
- Mental Health Act 1983 (as amended)
- Human Rights Act and safeguarding legislation
Skilled in multidisciplinary working and confident managing complex cases
Committed to strengths-based practice, inclusion, and independence

💼 What You’ll Need:

A recognised social work qualification (DipSW, CQSW, BA, MA in Social Work)
Excellent communication and decision-making skills
A flexible, proactive approach and the ability to adapt in a fast-paced environment
