Job Opportunity: Social Worker – RCT Hospital Discharge Team

Are you a skilled Social Worker with expertise in complex discharge assessments? Network Recruitment Wales is recruiting for a Social Worker to join the RCT Hospital Discharge Team for a 9-week contract. This is a fantastic opportunity to make a difference in supporting complex discharges and ensuring positive outcomes for patients.

Role: Social Worker (Hospital Discharge Team)
Location: Rhondda Cynon Taf 
Contract Duration: 18/08/2025 – 18/10/2025
Hours: 37 hours per week (Monday–Friday, 09:00–17:30, full-time only)
Salary: £29.13-£38.43 per hour

Key Responsibilities:
  • Conduct complex discharge assessments for the D2RA Pathway 3.
  • Apply knowledge and skills in the Mental Capacity Act and Continuing Health Care Funding.
  • Work collaboratively with hospital teams and community services to facilitate safe and effective discharges.
  • Manage a varied caseload with a focus on person-centered care.
  • Agile working between home and hospital sites as required.
Requirements:
  • Must be registered with Social Care Wales (SCW).
  • Car driver essential with the ability to travel between hospital sites in RCT.
  • Proven experience in complex discharge assessments and hospital-based social work.
  • Strong knowledge of the Mental Capacity Act and Continuing Health Care Funding.
  • Excellent organisational and communication skills to manage complex cases.
