We are recruiting for experienced Qualified Social Worker to work in a hospital team in Redbridge.
You will work within a multidisciplinary setting, discharge to assess model of work and within the current hospital discharge guidance. The social worker is expected to respond to referrals and arrange services that ensures the safety and wellbeing of the prior to their discharge from hospital.  Undertake Community Care Assessment and Review the services. Social worker is expected to have safeguarding experience, have good assessment and communication skills. SW will be offered supervision and support with case management
