Pay rate, £33 per hour  
The Talent Crowd are pleased to be recruiting for an experienced qualified Adult Social Worker to join a thriving Peripatetic Team, based across the county.
 
You will be required to support with triaging prior to patients leaving hospital, complete reviews including DST’s following hospital discharge as well as covering duty to manage urgent tasks as they arise.
