Adults Social Worker, Transitions Team, Surrey  
Up to £38.69 per hour 
To provide professional social work support to individuals, children, young people, families and carers living in Surrey in line with the requirements of current national legislation and local policies and procedures. They engage with individuals, children, families, groups and communities to assess and intervene and using professional judgement employ a range of interventions promoting choice, control and independence. The Transition Team has offices based in Woking and Redhill and is a countywide service for those aged between 14 and 25. We aim to give them the confidence and environment to control their life outcomes and aspirational goals that will support them into employment pathways, independent living and meaningful occupation. As a Social Worker in Surrey you will work across various unique projects that ensure our valued service users have the opportunity to enjoy a home of their own, an environment in which they are supported to live a health & meaningful lifestyle.
