Adult Social Worker, Older Persons Team, Hertfordshire 
Pay - £32 per hour.  
Based in the locality – covering relevant areas 
• Supporting reducing the risk of admission to hospital 
• Assessing/reviewing individuals in line with the Care Act 
• Arranging appropriate care and support 
• Undertaking Safeguarding enquiry officer role 
• Completing Mental Capacity and BI decisions  
• Working with other professionals
