Adult's Social Worker, Senior Practitioner, Transfer of Care, North London 
Pay rate – £36 per hour 
Contract role – Transfer of Care Team 
 
Pertemps are recruiting for experienced Adult's Social Worker in North London. 
This is to work in a Transfer of Care Team. 
The post is a Senior practitioner who will need to be on site a minimum of two days per week at Homerton Hospital as they will be leading on the ASC representation within the newly created Transfer of Care hub. 
 
There is plenty of hybrid working available.
