You will play a vital role in enabling people to live safely and independently in their homes through comprehensive functional assessments, specialist equipment provision, and major adaptations.
Key Responsibilities

- Complete manual handling assessments and develop safe handling plans involving hoists, slings, and transfer aids.
- Prescribe and oversee major home adaptations (e.g., level access showers, stairlifts, ramps, through-floor lifts) in collaboration with housing and DFG teams.
- Provide clinical reasoning and justification for high-cost equipment and adaptations, ensuring compliance with legislation and best practice.
- Liaise with contractors, surveyors, housing officers, and other partners to ensure safe and effective delivery of adaptations.
- Support timely hospital discharge and avoidance by ensuring provision of essential equipment and adaptations.
- Maintain accurate, professional documentation, contributing to safeguarding enquiries, legal reports, and funding panels.
- Offer peer support and supervision to junior staff, championing safe practice and reflective learning.
- Act as a key contact for service users, offering advice, advocacy, and reassurance throughout the adaptation process.
- Strong communication skills β able to explain complex clinical and technical information clearly.
- Organised and self-motivated β proactive in managing workload and personal development.
- Time management β able to work effectively to deadlines within structured processes.
- Flexible and accountable β responsible for agreed tasks and outcomes, working closely with managers.
- Problem solver β able to bring forward challenges, implement solutions, and review effectiveness.
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
