Adult Social Worker – Hospital Team -  Telford and Wrekin 
Pay Rate - £34 per hour
Based at Princess Royal Hospital 
Hospital team base/client visits/home.  
Qualified social worker who has experience in enablement services who can complete Enablement reviews and care act assessments, Mental Capacity Act assessments and Best Interests. Experience of asset based approach, completing CHC checklists, person centred approach and used to fast pace work. For a period of 3 months approx. it would be advisable they have experience of LAS.
