- Undertake assessments, reviews, and support planning to achieve personalised outcomes.
- Provide practical assistance, including access to living aids and advice on welfare benefits.
- Arrange short-term care and respite services to support carer well-being.
- Work closely with adult clients, carers, and communities to clarify needs and develop effective support plans.
- Liaise with health and social care partners to gather necessary information for assessments and plans.
- Maintain accurate case notes and reports, adhering to data protection and record-keeping protocols.
- Assist social workers in collecting and presenting information for legal cases.
- Undertake reviews of care packages and contribute to Multi-Disciplinary Team meetings.
- Attend safeguarding strategy meetings as needed.
- Complete Continuing Health Care (CHC) checklists and identify cases requiring full assessments.
Required Knowledge, Skills, and Qualifications:
- Genuine motivation for a career in adult social care, demonstrated through experience or exposure to the field (voluntary, paid, or personal).
- Understanding of adult social care’s role in promoting independence.
- Knowledge of social care ethics and values, with behavior aligned to these principles.
- Strong relationship-building skills to empathize, motivate, and support residents and carers.
- Creativity in identifying and implementing strength-based solutions for residents.
- Effective written communication skills for assessments, plans, and reports.
- Strong organizational skills to manage workloads and meet deadlines.
